If your physician provides services at one of the following IHP contracted facilities: (If they DO NOT provide services in any of the facilities listed below, please return to the Home page or New to PSIP).

• Centinela Freeman Regional Medical Center
• California Hospital Medical Center
• Downey Regional Medical Center
• Lakewood Regional Medical Center
• Long Beach Memorial Medical Center
• Memorial Hospital of Gardena
• St. Francis Medical Center
• White Memorial Medical Center

THEN, they may enroll in the PSIP-IHP Program. Please follow these instructions:

1. Click on the Director’s Letter button. This letter highlights the Reimbursement Policies and the time frames that are being considered for payment.
2. Next, click on the Billing Procedures and Reimbursement Policies and download the information. These documents will provide more detail on how to submit your claims.
3. Download and complete the forms indicated below.
   • Conditions of Participation
   • Provider Enrollment Form
